Gamepad key assignment change on the PC?

Problem 1

Due to a disability, it is necessary for me to swap the key assignment of any controller as I only have one hand available and play with my mouth. So far I've done this with a PS4 controller and InputMapper and DS4Windows. Everything worked fine. Suddenly it doesn't work anymore. In the program itself I can test whether the controller works and see the stops. Everything is displayed correctly there (I press R1 and the program emulates that I press L2) but in-game (e.g. CoD MW or Destiny 2) no longer works.

Only the start button and the touchpad work

I've made all the "fixes" on the subject that I could find on Google. I tested every USB port, used a second controller, a different USB cable, reinstalled drivers, and so on. All without success. It worked again in between and then it didn't work again. In the end I even reinstalled Windows 10 without success.

Problem 2

Now I got an Xbox One controller and connected it via USB and the controller works fine by default. Problem, I have to swap LT and RB or I can't aim. When I press RB it should be emulated that LT is pressed. But I don't find any software that enables this. Unfortunately, the XBox accessories app does not offer special reprogramming. I hope someone can help me.

I don't care whether I can get the PS4 controller back with InputMapper or the XBox controller with any software.

The app is not designed for this. In the app you can only swap LB / LT and RB / RT completely. But that doesn't do me any good because then I would still shoot on the left and aim on the right side of the controller. I have to swap the left trigger and the right shoulder button (xbox controller: LT with RB or PS4 controller L2 with R1). InputMapper gave me the opportunity to do so, but it no longer works.
