Ps4 score transferred to the PC?

I played the Witcher 3 on the PS4 2 times (NewGame +) and I would like to transfer my score when it's gait I have both dlcs Heart of Stone I've been through and blood and wine up to half aprox.

Is there perhaps a way to transfer this score to the PC?
