Which open world games, where you can design the character yourself (outfit and look) are there for the PS4?

The outfits should not be provided with skills etc. As with DQ IX or Fallout, so that you can wear your desired outfit without disadvantages.

I'm particularly interested in games from the (classic) fantasy sector.

Definitely Final Fantasy 14 would be the first for you. I think 30 days free and then it costs 10.99 euro per month & is an MMO but with a very, very good story, especially in the last package "Shadowbringers"
