I got the PS app on my phone and set a picture as a profile picture and hold an avatar. Can I somehow set this image to be displayed as an avatar on Ps4?

You can't make your profile picture public unless you have Sony VIP or "Star Gamer" status. Otherwise they have to send you a "real name friend request".
