To 1.
You open the eShop on the Nintendo Switch, there's the point "Redeem code". There you then enter the download code and download the game into the internal memory or the SD card.
To 2.
"Nintendo Switch Online" is a paid service which you can use to download some classic Nintendo games for free to play.
The service for the online functions of some games is also required.

In ACNH you need it e.g. To visit other player islands via the online function or to invite other players to your own island.
You can also do this via the local mode, but you have to meet these people in real life. No Nintendo Switch Online is required for this.
In principle, you can also play ACNH without a Nintendo Switch Online membership.

I have no experience on the subject.

You enter this code in the e-shop. Then Animal Crossing will download (takes quite a long time (at least that was the case with me))
You only need online membership if you want to fly to other islands from other players, e.g. To play or trade with friends. But it is not a must to buy them
It's called an island per switch, so nothing is saved.
