Twitch affiliate, specify correct data?

So now I've met the requirements for Twitch's affiliate program. Now I have to specify my complete personal data, where I have little desire on it. Can I just give something else, so different name, city, Starsee… And so I get mail from Twitch or the tax office? And what about the tax return? Do the income have to be listed there?

Are you serious? Then do not use the program & it's good.
What you also have to specify is your tax ID. Without them you will not get ahead anyway because Twitch needs this to make a corresponding message to the tax office.
