Do you have anything in there that contained the bundle? For example, a screenshot of the bundle display? Codes that are in a sealed XBOX package do not require activation. In your case, I would first contact Microsoft. If they can't help, go to the dealer where you got the encoder from. This must then endeavor to send you a new code.
Unfortunately not about the content. The stupid thing is that all other codes were in the Xbox packaging and the Live Gold on the outside of the Xbox was stuck to a tab inside. This tab even says that you have to activate it at the checkout. However, these bundles are series production.
