XBox one s disk?

Have an xbox one s and forza horizon 4

Everything installed CD is new and clean but sometimes the xbox says something like disk not recognized make sure that it is clean or scratch-free. What could be the reason? Xbox used but almost never used as good as new

This could be due to the disk drive. It could be that the drive is dusty or has an error. You could also unscrew your Xbox and replace the drive (only a recommendation if the problem might be that the drive is damaged)

Is that hard to switch?

Not really. Remove the Xbox from the power (pull the cable), remove the cover with a screwdriver, disconnect the drive cable, unscrew and screw in a new drive and plug the cable back in
