I'm looking for games that combine characteristics typical of their genre. The best would be an open world in which you can significantly influence the game through your own decisions. In addition, you should be able to design your hero yourself if possible and control him best in the third person (so please no typical building games that only run over the bird's eye view). I think of Mount & Blade 2 as an example, but unfortunately it is not available for the PS4. The epoch or the setting are rather unimportant here, so it does not necessarily have to be in the Middle Ages.

Could also recommend Dark Souls but it only has different endings!
How about Kingdom Come: Deliverance?
You start small and have to master all your abilities before you really have a chance.
However, I'm not sure whether you can play the game in third perspective.
