Streaming gear PS4?

I want to start streaming on Twitch. I want to broadcast games live on PS4 using a PC.

I'm looking for recommendations for:

a camera
a microphone
a capture card (if needed)
a computer

Everything should be affordable and not overly expensive. First I want to see if I like streaming at all.
