You need one monitor per monitor from the graphics card to the monitor.

You plug the second monitor into the next available slot on the graphics card. Whether that is now an HDMI, VGA or even DVI connection is irrelevant. In the display settings of Windows, you can then adjust the resolution and placement. No free outputs means no second screen.
If you have no other output on the graphics card but absolutely want to connect a second screen, you have no choice but to buy a new graphics card.
