I have an Xbox Live subscription with a monthly charge. This expires on 31.1.19. Now I want to buy online a 12 months subscription per code.

If I activate this subscription, it starts immediately, or only from 1.2. After expiry of the old subscription?

After expiry of the old subscription. If you have an active subscription, everything you charge will be "on top"
